Every three years, medical examiners re-register for approval as an Element NL medical examiner. Use this form to register for the period 2027-2029. We will review your registration and then contact you.

5 Publication and declaration
Do you agree to the publication of your details on the list of Element NL medical examiners on the Element NL website?

Element NL processes the data you provide in accordance with the General Data Protection Regulation (GDPR).

By signing this form digitally, you declare:

  • that you have taken note of NOGEPA Standards 10, 11 and 12, endorse them and apply the most recent version as the standard for occupational health examinations;
  • that you will attend a refresher training every year;
  • that you will cooperate with any audit of your facilities and examination activities for compliance with the NOGEPA Standards, commissioned by Element NL;
  • that you agree to the fees Element NL charges for registration as an Element NL medical examiner;
  • that you will complete future activities that Element NL may ask of you in the interim (such as e-learnings).
